Regular Cleaning (Prophylaxis)
A regular cleaning sometimes called a prophylaxis is the routine cleaning most patients are familiar with. It’s recommended every six months for patients with healthy gums and no signs of periodontal disease.
During a regular cleaning, your hygienist will:
- Remove plaque and tartar from the surface of the teeth and just below the gumline
- Polish teeth to remove surface stains
- Floss between teeth
- Check for early signs of decay or gum irritation
This cleaning is preventive. It’s designed to keep healthy gums healthy and catch small problems before they become big ones.
Deep Cleaning (Scaling and Root Planing)
A deep cleaning, clinically known as scaling and root planing, is a more involved treatment used when plaque and tartar have built up below the gumline, often as a result of gum disease (periodontitis).
Deep cleanings involve two parts:
- Scaling – Removing plaque and tartar from the tooth surface and root, well below where a regular cleaning reaches
- Root planing – Smoothing the tooth root so gums can reattach and heal, reducing the pockets where bacteria collect
Because this reaches deeper tissue, deep cleanings are usually done in sections (often split into two visits, treating one half of the mouth at a time) and may involve local anesthesia to keep you comfortable.
How Do You Know Which One You Need?
This comes down to one main factor: the health of your gums, specifically how deep the pockets are between your teeth and gums.
- 1-3mm pockets, no bleeding or inflammation → Regular cleaning is sufficient
- 4mm+ pockets, bleeding, swelling, or bone loss → Deep cleaning is typically recommended
During your exam, Dr. Mendu measures these pockets with a small probe as part of a periodontal evaluation. It’s quick, painless, and gives a clear picture of what level of cleaning your gums actually need.
Signs You Might Need a Deep Cleaning
- Gums that bleed easily when brushing or flossing
- Persistent bad breath
- Gums that look red, swollen, or are receding
- Teeth that feel loose or like your bite has changed
- It’s been years since your last cleaning

Does a Deep Cleaning Hurt?

What Happens After a Deep Cleaning?
Unlike a regular cleaning, a deep cleaning usually comes with a follow-up plan. This might include:
- A follow-up visit to check healing and pocket depth
- More frequent cleanings (often every 3-4 months) called periodontal maintenance
- At-home care recommendations specific to your gum health
The goal is to stabilize your gums and prevent the disease from progressing to more advanced and harder to treat stages.
Why This Distinction Matters
Skipping a recommended deep cleaning in favor of a regular one won’t address the actual problem below the gumline. Gum disease doesn’t resolve on its own, and the longer it goes untreated, the more bone and tissue support you risk losing.
The reverse is also true: not everyone needs a deep cleaning. If your gums are healthy, a regular cleaning is all that’s necessary, and there’s no reason to do more than that.
